Important Blue Cross Tailored Network Information: 

If your insurance provider is a Blue Cross Tailored Network (bought through the healthcare exchange). To schedule an appointment with us we need to be your primary care provider (PCP) or have a referral from your primary care provider (PCP). If your insurance card says “HometownEast” on it than we can be your PCP. With any other Blue Cross Tailored Network holders, you can still be seen by us, you just need to get a referral from your PCP. If you do not know who your PCP is, call the number on the back of your Blue Cross card.
